Use this form ONLY to add more summer sessions, after previously submitting the 2019 Summer Registration form. This form does not include the personal information that we require about your child (e.g.: allergies, emergency contact info); it is only to add more programs.
If you complete this form before you complete the 2019 Summer Registration Form, we'll have to contact you to restart the whole process. That's no fun for you or for us!

SUMMER CANCELLATION POLICY
UAS programs require a high degree of research, planning, logistics, and care. We use registration numbers and fees to hire/confirm staff, and we work diligently to make sure that every program we deliver is both high quality and as reasonably priced as possible.
For these reasons, we cannot offer refunds any later than April 27, 2019. Cancellations prior to Friday, April 26 will incur a cancellation fee of $100 per child, per session. If you need to cancel your registration by April 26, we will issue you a refund of $235, and $168 per sibling.
Please note that we must have a minimum of 15 confirmed/paid students for each week-long program by March 1, 2019, in order to run the program. In the event that we must cancel an under enrolled program, we will issue families full refunds. We have never had to cancel a summer week, but we want to be transparent about our policies and the importance of being able to cover our costs.
And remember, registration is a three-step process:
1. Fill out and submit this registration form and click "Submit." You'll be automatically redirected to a page acknowledging that we have received your form.
2. We'll confirm that we have space in the session(s) you've signed up for, and a member of the UAS staff will send you payment instructions from our general email address, info@urbanadventuresquad.org (please add this address to your contacts). Please be patient. We have a very small staff and this can take a few days.
3. After you have successfully submitted payment, your registration is complete!
